2. Costumes will NOT be ordered if your dance account is not up to date. Costumes ordered late will have a separate shipping charge. Costumes will NOT be sent home until the costumes are paid for and your account is up to date. A $40.00 non-refundable costume deposit (per class) will be due in November.
3. There are no credits or refunds allowed for missed classes. Classes missed by students may be made up once approved by the instructor.
4. The Hampden Performing Arts Centre reserves the right to substitute instructors. In the case of the instructor cancelling a lesson, a make-up class will be arranged. Classes missed due to inclement weather will not be made up.
5. Dress & Tech Rehearsals are Mandatory in order to participate in the recital! Class tuition and costume fees will not be reimbursed if rehearsals and recital are missed.
General waiver of liability
I the parent/guardian understand that dance and exercise programs require physical exertion and that
physical injury may occur. Despite this risk, I am voluntarily allowing my child to participate in these activities and will assume all risks involved.
I release and hold harmless the Hampden Performing Arts Center and their instructors from any and all
liability, claims, demands and causes of action related to injury while at the dance studio or at the facility
where we hold the HPAC recital and rehearsal practices.
I the parent/guardian understand from time to time dance photos may be shared on social media and in
advertising for promotional purposes. I hereby waive any right to inspect or approve any image.
